WHY: Young people with a mental illness and no adult support system face the same challenges as other youth when they transition into adulthood, plus bear the additional burden of an invisible disability. The Youth In Transition Program provides comprehensive mental health treatment, life-skills training, and vocational and case management services to youth 14 to 25 years of age who have been diagnosed with a mental illness, in order to build an essential bridge to responsible adulthood.
Founded as a private, nonprofit treatment center in 1957, Community Reach Center, formerly Adams Community Mental Health Center, was the first community mental health center to be established under the Community Mental Health Center's Act in the Rocky Mountain Region, and one of the first in the U.S. It is one of the premier mental health providers in the north metro Denver area, providing evaluations, counseling and therapies to adults, adolescents, children, families and seniors at locations in Brighton, Commerce City, Northglenn, Thornton and Westminster.
