What:	At Centura Health at Home, we are deeply committed to supporting families in keeping their loved ones at home. The Adult Day Program at High Street (near I-25 & University) provides a unique and attractive community based day care setting that will enhance the quality of life for your loved ones. In addition, you will experience peace of mind in knowing that your loved one is in a safe, secure, social, and peaceful environment. Daily Activities include: -Exercise Therapy -Crafts -Cooking Classes -Cognitive Exercises -Horticulture Therapy -Aromatherapy -Music Therapy -Community Outings -Seasonal Events We also offer: -Continental breakfast, hot lunch and afternoon snack -Opportunity for socializing with peers and building relationships -Assistance with toileting, transfers, and other activities of daily living. -Access to telehealth monitoring services -Full activities assessment done on admission -Ongoing health education provided by Centura Health at Home staff -Annual friends and family events
