Each year, more than 7,000 people in Colorado suffer a stroke, the majority being from a blood clot, or an ischemic stroke. Having an ischemic stroke is the strongest predictor of having another stroke in the future. Almost 30% of strokes are recurrent strokes and 25% to 40% of patients have a recurrence within five years.
Colorado Neurological Institute is eager to address these staggering numbers through an innovative program, being funded by the Cancer, Cardiovascular, and Pulmonary Disease (CCPD) Grants Program within the Colorado Department of Public Health and Environment.
This three year grant, in conjunction with Swedish Medical Center, will target ischemic stroke patients brought into Swedish Medical Center by introducing risk-reducing medications and behavioral interventions in the acute-care hospital setting, followed by intensive post-discharge follow-up to monitor and encourage patient compliance. In years two and three of this project CNI will also be working with our partner hospitals with CO-DOC, Colorado Digital Online Consultant, telemedicine project, previously funded by the CCPD Grants Program to offer the same follow-up to stroke patients in rural Colorado.
